Certification of Finances Questionnaire

Newly enrolled international students must complete a Certification of Finances questionnaire to certify that they have financial resources adequate to provide for their educational expenses, and upload the verification of finances document (bank statement or letter) through the Terra Dotta student portal.  Access to the Terra Dotta portal will be granted after an applicant enrolls at SVA.  Please note that these documents are required in order to be issued an I-20.

I-20 Transfer Form

Submit this form only if you currently attend another school in the U.S. and have a valid I-20.

If you already have a valid F-1 visa or status and are enrolled in another SEVIS approved institution in the U.S., you must have your I-20 sponsorship transferred to SVA.  Please submit the I-20 Transfer form only after you have been accepted to SVA and have paid the enrollment fee.

  • Download the I-20 Transfer form.
  • Have the international student advisor at your current or previous school fill out and sign the form. Return the form to SVA's International Student Office with a photocopy of your passport and I-20 issued by the school you have last attended.
J-1 Exchange Visitor Health Insurance Requirement Form

F-1 students authorized by USCIS to engage in Optional Practical Training (OPT) are required to report any change of name, address, or employment status to the International Student Office within 10-days. Please submit this online form to report the changes.
