Certification of Finances Questionnaire

Newly enrolled international students must complete a Certification of Finances questionnaire in the Terra Dotta portal to certify that they have financial resources adequate to provide for their educational expenses and upload the verification of finances document (bank statement or letter).  Access to the Terra Dotta portal will be granted after an applicant enrolls at SVA.  These documents are required in order to be issued an I-20.  

Access to the Terra Dotta system is granted only to students upon enrollment.  An email invitation to complete the program will be sent to the SVA email account shorty upon enrollment.

I-20 Transfer Form

Submit this form only if you currently attend another school in the U.S. and have a valid I-20.

If you already have a valid F-1 visa or status and are enrolled in another SEVIS-approved institution in the U.S., you must have your I-20 transferred to SVA.  Please submit the I-20 Transfer form only after you have been accepted to SVA and have paid the enrollment fee.

  • Download the I-20 Transfer form in the Terra Dotta portal.
  • Have the international student advisor at your current or previous school fill out and sign the form. Return the form to SVA's International Student Office by uploading a scan to the Terra Dotta portal.
J-1 Exchange Visitor Health Insurance Requirement Form

This form must be submitted by all J-1 exchange visitors and dependents before the start of the semester.  The form is mailed with the DS-2019.

SEVP Portal for Reporting OPT Employment Status

Effective March 23, 2018, OPT employment updates must be made directly via the SEVP Portal.

F-1 students authorized for Optional Practical Training (OPT) are required to report any change of name, address, or employment status to the SEVP Portal within 10-days.  Change in employment information during the 24-month STEM OPT extension must be reported by submitting an updated I-983 form directly to SVA/ISO.
