Confidential Declaration and Certification of Finances (DOF) Form

International applicants are required to submit a declaration of finances (DOF) indicating that the applicant has financial resources adequate to provide for his or her educational expenses.  The amount declared must reflect the total sum of tuition, fees and living expenses for one academic year of study and must be dated within one year of initial enrollment at SVA.

Verification of Finances

All funds declared on the DOF must be substantiated by a verification of finances (VOF) letter from an officer of the student's bank or sponsor's bank.  Funds must bein a readily accessible form such as liquid assets.  All documents must be in English and in U.S. dollars, or the current exchange rate must be supplied by the bank.  The DOF and VOF must be completed and returned to the Office of Admissions.

I-20 Transfer Form

Submit this form only if you currently attend another school in the U.S. and have a valid I-20.

If you already have a valid F-1 visa or status and are enrolled in another SEVIS approved institution in the U.S., you must have your I-20 sponsorship transferred to SVA.  Please submit the I-20 Transfer form only after you have been accepted to SVA and have paid the enrollment fee.

  • Download the I-20 Transfer form.
  • Have the international student advisor at your current or previous school fill out and sign the form. Return the form to SVA's International Student Office with a photocopy of your passport and I-20 issued by the school you have last attended.
J-1 Exchange Visitor Health Insurance Requirement Form
