FREE and CONFIDENTIAL counseling services at SVA

Student Health and Counseling Services provide free of charge, confidential therapy for enrolled students at SVA. We are open Monday to Friday, 9 AM to 5 PM. You may request an appointment by clicking the button below or call 212.592.2246 during our office hours.

Student Health and Counseling Services does not write medical excuse notes.

Students who are not able to attend classes due to illness should notify their instructors and, for conditions requiring an extended absence from classes, their Academic Advisor. For academic accommodations due to physical or mental health, please see Disability Resources.

Absences to illness are not excused; students who are ill should not provide doctor's notes to faculty members, nor should a faculty member accept such documentation from a student.

This is also noted on page 12 of the SVA Student Handbook.

Incoming Students

SVA Student Health Form

This form is required of all incoming/accepted students that want to enroll in classes at SVA. NOTE: If you attended a SVA Pre-College course and submitted a health form -or- if you are a BFA SVA Alumnus, please e-mail to check if we have your immunization record on file.

Students can complete the top half of the form and Step 1-Meningitis Response; a clinician / health care provider must complete Step 2-Measles, Mumps, Rubella*

*We will also accept a copy of a student's health records displaying proof of MMR (2) Measles, (1)  Mumps and (1)Rubella vaccinations in place of Step 2. Students who attended a SVA Pre-College course and previously submitted the health form or obtained a BFA from SVA do not need to re-submit this form. Contact to inquire.

The New York State Department of Health Immunization Program has the legal authority to ensure that schools throughout the state comply with PHL Sections 2165 and 2167No institution shall permit any student to attend such institution in excess of thirty days without complying.

All students are automatically enrolled and billed for the SVA-sponsored insurance plan, Cigna PPO. It is an accidental injury and sickness plan. This plan does NOT include dental or vision.

Students Leaving SVA for Medical Reasons / Returning to SVA From A Medical Leave

Physician's Verification of Medical Condition Form: Students Leaving SVA for Medical Reasons 

Students that take a Medical Leave of Absence from SVA must submit this completed form to Student Health and Counseling Services no later than 30 days from the date of withdrawal for tuition consideration. 

Certificate Of Readiness Form: Students Returning to SVA from a medical Leave  

Students that took a Medical of Absence from SVA and want to request a return must submit this completed form to Student Health and Counseling Services at least 1-2 months from the date of the anticipated semester's start.

Meet Our Staff

Christine Gilchrist, MSN, MPH, RN, NC-BC

Associate Director, works at SVA Monday-Thursday

Christine has been at SVA since 2002. She is an integrative nurse; using a whole person/whole system approach to caring for others. She was a sexual health peer educator in college; majoring in Nursing and minoring in Women's Studies at the University of Pennsylvania. Her graduate degrees are in Community Health Nursing and Urban Public Health with a focus on Community Health Education. Christine is a board certified nurse coach, working with SVA students on setting and achieving wellness goals. She also teaches mind/body techniques such as breathing and meditation for stress management.

Arielle Kempler, LCSW 
Therapist, works at SVA Monday-Friday

Arielle is a full-time psychotherapist who joined SVA in 2018. She specializes in trauma therapy and has previous experience working with survivors of domestic violence and sexual assault. She approaches therapy with a trauma perspective and integrates theories such as Internal Family Systems theory (IFS) and somatic experiencing in her work. She also uses techniques that connect the mind and body such as mindfulness and deep breathing to help target anxiety. Her practice focuses on helping clients overcome barriers in their lives that prevent them from feeling in control and achieving their goals.

Brett Sharetts, LCSW 
Therapist, works at SVA Monday, Tuesday, Thursday Friday (East Side), Wednesday (West Side)

Brett is a full-time psychotherapist who joined SVA in 2018. He approaches therapy with a modern psychodynamic perspective, integrating theories from psychoanalysis, CBT, and mindfulness based practices. His goal is to help clients understand their own process and unconscious motivations as they strive to live fuller, richer, more meaningful lives.

Holly Fu, LMSW

Therapist, works at SVA Monday-to-Friday

Holly is a full-time psychotherapist who joined SVA in 2019. She specializes in interpersonal therapy and has extensive experience working with clients from various social-economic, cultural and ethnic backgrounds. She approaches therapy with a psychoanalysis perspective and focuses on personal growth. Her goal is to empower clients to be in charge of their lives and live a more fulfilling and meaningful life.  She speaks both English and Mandarin. 

Daniela Caraballo, LCSW

Therapist, works at SVA Monday-to-Friday

Daniela is a full-time psychotherapist who joined SVA in 2019.  She’s worked with children, adolescents, and adults as a Licensed Clinical Social Worker in a range of mental health settings. She approaches therapy with an integrative framework of various therapeutic techniques including, Cognitive Behavioral, Relational, Psychodynamic, Family Systems, trauma focused and mindfulness based practices. She identifies as an LGBTQI affirming therapist and adopts an anti-oppression/anti-racist stance in her work. Her goal is to help clients make meaningful connections and learn tools to tackle obstacles preventing them from living a meaningful and fulfilling life. She is a native New Yorker, fluent in English and Spanish.

Dr. Mark Howell, LCSW

Therapist, works at SVA - WEST SIDE - Tuesday and Friday

Mark Howell, LCSW has been with SVA since 2000. He specializes in social work and clinical social work.

Matt Lee
Coordinator, works at SVA Monday-Friday
Matt has been at SVA since 2015. He schedules appointments for SVA students, processes immunization forms and manages student health insurance requests.

1095-B Information
Purpose of IRS Form 1095-B: Health Coverage
Form 1095-B is used to report certain information to the IRS and to taxpayers about individuals who are covered by minimum essential coverage and therefore are not liable for the individual shared responsibility payment. Consolidated Health Plans mailed 1095-B's to students insured under the SVA sponsored health insurance plan in January 2019.
If you recently changed addresses in the past few months: You'll need to contact to obtain a copy.
January 2020: Students enrolled in the SVA sponsored health insurance plan for the 2019 year should check their online account for their 1095-B at the end of January 2020.

INTERNATIONAL STUDENTS: You only need to keep this form for your records and no action is required.
